Document provides a list of information to collect while obtaining medical history of a patient and conducting a physical examination. Also includes two envelopes with Drew's return address at Freedmen's Hospital, Washington D.C.
File consists of an athletic shirt owned by Drew with the McGill University crest sewn onto front and two first-place medals awarded to Charles Drew from the Canadian Intercollegiate Athletic Union, one for 120 yard hurdles (1932-1933) and one for shotput (1928).
One printed booklet: "this Supplement includes the names of the five hundred and fifty-four diplomates who were awarded the Certificate of the National Board of Medical Examiners during the year 1934". Charles Drew's name is found on page 5.
